By registering for this event, you agree to the following Participation Agreement:
In consideration for the opportunity to participate in the above activity, the Participant (or parent/guardian if Participant is a minor) acknowledges and accepts the risks of injury associated with participation in and transportation to and from the activity. The Participant (or parent/guardian) promises to hold harmless the activity sponsor, or its agents, employees, volunteers, or any other representatives (collectively referred to hereinafter as the “Sponsor”) for any injury related directly or indirectly out of the described activity or transportation to and from the activity. The various types of injuries include, but are not limited to, the following: sickness, exposure to infectious /communicable disease, bodily injury, death, emotional injury, personal injury, property damage, and financial damage. I also certify that photographs or videos of my student participating in any Kensington Church sponsored program and/or activity may be reproduced and utilized in promotional materials for the church. I understand that Kensington Community Church will not release my student's name or other personal information.
If a dispute over this agreement or any claim for damages arises, the Participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the Participant (or parent/guardian) and the Sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel of the American Arbitration Association for final resolution.
WHAT TO EXPECT
- Our in-person services will be a physically-distanced gathering. Individuals will be asked to abide by social distancing recommendations.
- A reservation system has been created to ensure safety and social distancing. Entry will require a confirmation email for each individual in your party. Once your spot(s) is reserved, your name(s) will be added to our guest list for a quick, no-scanning entry process – just state your name at check-in.
- All in attendance will be asked to wear a mask or face covering anytime inside our buildings.
· If you have experienced any COVID-19 symptoms or have been around someone exposed or potentially exposed, please stay home for the safety of others.
We take the greatest care to safeguard our guests, however participation in services and events is voluntary and your health is your own responsibility. By attending you are acknowledging the inherent risk of exposure to COVID-19 and you and any guests voluntarily assume all risks related to exposure to COVID-19 and agree not to hold Kensington or any of its elders, employees, agents, contractors or volunteers liable for any illness or injury.
